In consideration of being allowed to participate on behalf of Southern Chester County Sports LLC and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

​

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Southern Chester County Sports LLC, their officers, coaches, officials, agents, volunteers and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

5.  I understand that I am required to have accidental medical coverage for the child listed on this waiver, and I verify that the information provided on this form is accurate and true. I understand and agree that if I do not have accidental medical coverage for the child listed on this waiver, I will be financially responsible for all charges and fees incurred in the rendering of said treatment. In case of an injury, I authorize the Southern Chester County Sports LLC and its volunteers to render first aid. I understand that at the discretion of the Southern Chester County Sports LLC volunteers, my child may be dismissed from the team or camps without refund for inappropriate behavior. I understand that at the conclusion of the scheduled programs, Southern Chester County Sports LLC and its volunteers are no longer responsible for my child.

6.  I give permission to use, reprint and produce any photographs or videos taken of me or my child and written materials supplied by me or my child in the form of evaluation during the youth sports program. I hereby authorize the Southern Chester County Sports LLC Softball volunteers to act for me in case of an emergency and waive and release Southern Chester County Sports LLC Softball volunteers from any and all liability for any and all injuries and illness occurred while at involved in Southern Chester County Sports LLC activities.
